On September 3, 2014, at around 01:20 KST, in South Korea, the group's van ran into a protective wall in the vicinity of the Singal junction on the Yeongdong Expressway in the direction of Incheon. As a result of the accident, EunB died while the remaining six passengers were injured and rushed to the hospital.Two of the members, Sojung and Rise, were taken to the Catholic University of Korea St. Vincent's Hospital in Suwon, listed under critical condition. Sojung's condition has since been reported as stabilized, however Rise passed away in Ajou University Hospital four days after the incident on September 7, after being in a coma since the accident and undergoing numerous surgeries.

It was reported that none of the van's airbags deployed at the time of impact. By the time the paramedics arrived at the scene, Eun-bi was already in cardiac arrest while also suffering from an open skull fracture. She was declared dead on arrival (DOA) by the time the paramedics reached Sungbin Medical Center.[7]

Rise was immediately taken to Aju University Hospital, where she was given emergency CPR before being immediately taken into surgery due to suffering severe cranial and abdominal injuries.[24][25][26] The surgeons operated on her cranial area three times while also having to resuscitate her via defibrillation during the procedures.[27] In the midst of a fourth procedure for her back, [28] in her 11th hour of surgery, Rise's blood pressure began to drop very rapidly, making it unsafe to continue.[29] Thus, she was moved to the intensive care unit, where she remained in critical condition. It was reported that her brain was severely swollen and that she was not regaining consciousness, with the doctors closely monitoring her condition in order safely resume surgery.[30]
Rise eventually succumbed to her injuries at 10:10am (KST) on September 7, 2014 in Aju University Hospital, having never woken up from her coma.[31][24][32] Her wake was held at Korea University Anam Hospital funeral home.[33][34]
